Required Notifications

Amusement Ride and Tramway (ART) Unit

  • Apply for permits to operate temporary amusement rides.
  • Submit a route list/itinerary containing each operating location, dates of operation, and a list of rides in operation at each location for the permitting year.
  • Notify the Division of the intent to operate a new ride or perform a major modification to an existing ride.1
  • Report immediately by telephone any injury, accident, or major mechanical failure2 resulting from the maintenance, operation or use of a temporary amusement ride.

  • Notify the Division of intent to open a new permanent amusement ride or perform a major modification to an existing ride.3
  • Submit requests for the Division to conduct a QSI inspection on a permanent amusement ride.
  • Report an injury or accident resulting from the maintenance, operation, or use of a permanent amusement ride.4

  • For information, call the nearest ART Unit office handling permanent amusement rides.
  • Notify the Division of intent to install a new passenger tramway or perform a major alteration to an existing passenger tramway.5
  • Report an injury or accident resulting from the maintenance, operation, or use of an aerial passenger tramway.6

Asbestos and Carcinogen Unit

  • Report use of the carcinogen.7 Contact the Asbestos and Carcinogen Unit.

Crane Unit

  • Report deficiencies affecting the safe operation of a crane during the course of any certification inspection within 5 working days after the test date.8 Contact the Crane Unit.

Mining and Tunneling (M&T) Unit

  • Notify in writing of the start of certain construction and demolition activities at a new site.17 Contact the nearest district office of the M&T Unit.
  • Notify Cal/OSHA to obtain classification of environmental hazards in a proposed underground mine or proposed tunnel prior to bidding the project.18 Contact the nearest district office of the M&T Unit.
  • Notify Cal/OSHA to conduct a pre-job safety conference for an underground mining or tunneling operation prior to beginning the project.19 Contact the nearest district office of the M&T Unit.
  • Report excessive gas levels, fires, or other hazards in a mine or in a tunnel under construction.20 Call the nearest district office of the M&T Unit.

Pressure Vessel (PV) Unit

  • Report that insurance has been refused for an air tank21 or boiler.22 Contact the Northern or Southern California office of the PV Unit.
  • Report that an air tank23 or boiler24 has been removed from service or should be removed from service. Contact the Northern or Southern California office of the PV Unit.

Employers of Certified Inspectors:

  • Notify the Division when employment of a certified inspector is terminated.25 Contact the Northern California office of the PV Unit.

Process Safety Management Unit

  • Submit a full schedule of planned turnarounds every September 15 for all affected units for the following calendar year.26 Contact the Process Safety Management Unit.
