Workers' compensation pharmacy fee schedule - Simple prescription

NOTICE: The February 20, 2019 data file will remain in effect until the DWC has adopted updates to the Pharmaceutical Fee Schedule that are necessary in light of changes in the Medi-Cal Pharmacy Fee Schedule payment methodology. DWC will be holding a public hearing and accepting public comment on the fee schedule revisions. Weekly updates to the data file will resume after the revised fee schedule methodology is adopted. Notices relating to the Pharmaceutical Fee Schedule rulemaking action will be sent to the DWC Newsline mailing list. An NDC number is either 10 or 11 digits, made up of three segments (manufacturer/distributor, drug identifier, package code), with the number of digits in each in segment in one of the following patterns:
11 digit: 5-4-2
10 digit: 4-4-2, 5-3-2, or 5-4-1

NDC number: Please include the correct number
of leading zeros, e.g. 0093 0014 16
e.g. 100 ea (tablets), 2.5 grams (ointments), 240 ml (or cc's - fluids)
(including any dispensing fee) e.g. $12.48
e.g. 01/05/2004

Workers' compensation pharmaceutical fee schedule — Physician dispensed pharmaceuticals

Legislation that became effective on January 1, 2012 places new caps on fees for pharmaceuticals dispensed by physicians. These new caps are contained in Labor Code section 5307.1.
