Amusement Ride and Tramway (ART) Unit
Temporary Amusement Ride Owners and Operators must:
- Report an intention to erect a new ride or alter an existing ride1. Contact the ART Unit office handling temporary amusement rides.
- Report an injury, accident, or major mechanical failure2. Call the ART Unit office handling temporary amusement rides.
- Report an intention to operate a new permanent amusement ride or reopen a permanent amusement ride after a major modification3. Contact the nearest ART Unit office handling permanent amusement rides.
Permanent Amusement Ride Owners and Operators must:
- Report an injury or accident on a permanent amusement ride4. Call the nearest ART Unit office handling permanent amusement rides.
Tramway Owners and Operators must:
- Report an injury or accident on an aerial passenger tramway5. Call the ART Unit office handling .

Asbestos and Carcinogen Unit
Employers Using a Carcinogen must:
- Report use of the carcinogen6. Contact the Asbestos and Carcinogen Unit.

Crane Unit
Crane Certifiers and Derrick Certifiers must:
- Report deficiencies affecting the safe operation of a crane during the course of any certification inspection within 5 working days after the test date7. Contact the Crane Unit.

- Notify in writing of the start of certain construction and demolition activities at a new site10. Contact the district office that serves the job location. For mining and tunneling activities, contact the nearest district office of the Mining and Tunneling (M&T) Unit.
Employers that are Registered to Work with Asbestos must:
- Notify in writing the start of asbestos-related work11. Contact the district office that serves the job location.
Employers Doing Lead-Related Construction Work must:
- Notify in writing of the start of lead-related construction activities12. Contact the district office that serves the job location.
Employers Using a Carcinogen must:
- Report a carcinogen release incident13. Contact the district office that serves the job location. Also contact the Asbestos and Carcinogen Unit.

Mining and Tunneling (M&T) Unit
Employers must:

Employers Holding an Annual Permit must:
- Notify in writing of the start of certain construction and demolition activities at a new site16. Contact the nearest district office of the M&T Unit.
Owners must:
- Notify Cal/OSHA to obtain classification of environmental hazards in a proposed underground mine or proposed tunnel prior to bidding the project17. Contact the nearest district office of the M&T Unit.
Operators and Contractors must:
- Notify Cal/OSHA to conduct a pre-job safety conference for an underground mining or tunneling operation prior to beginning the project18. Contact the nearest district office of the M&T Unit.
- Report excessive gas levels, fires, or other hazards in a mine or in a tunnel under construction19. Call the nearest district office of the M&T Unit.

Pressure Vessel (PV) Unit
Inspectors Employed by an Insurer must:
- Report that insurance has been refused for an air tank20. Contact the northern or southern California office of the PV Unit.
Inspectors Employed by Other than an Insurer must:
- Report that an air tank has been removed from service or should be removed from service21. Contact the northern or southern California office of the PV Unit.

Process Safety Management Unit
Petroleum Refinery Employers must:
- Submit a full schedule of planned turnarounds every September 15 for all affected units for the following calendar year22. Contact the Process Safety Management Unit.
